Please also tell us about any changes to your health before dental treatment, including new or changed medicines, allergies, medical conditions, recent or planned operations, or hospital treatment. Bring an up-to-date list of your medicines to your appointment.
Contact the practice or speak to reception or your dentist to update your details. Please do not enter medical information into this app.
If you cannot attend your appointment, please give us at least 24 hours’ notice.
Patients who cancel with less than 24 hours’ notice on two consecutive occasions may lose their NHS place at the practice and may no longer be offered NHS appointments.
We understand that genuine emergencies and exceptional circumstances can happen, and these will always be taken into consideration.
